Below are several forms that are used by G&R. Please feel free to download these files. They are here to help you do business with us.
The following forms and reference information are provided for our subcontractors and vendors. Feel free to download these forms for your review and use. We encourage our vendors and subcontractors to check with our Project Manager if you have any questions concerning these forms.
Lump Sum Subcontract
This is a copy of Gibbs & Register's lump sum subcontract. This subcontractor is used for lump sum contracts that exceed $20,000.00.
Unit Price Subcontract
This is a copy of Gibbs & Register's unit price subcontract. This subcontractor is used for unit price contracts that exceed $20,000.00.
Trucking Subcontract
This is a copy of Gibbs & Register's trucking subcontract. This subcontractor is used for all trucking/hauling contracts.
Short Form Subcontract
This is a copy of Gibbs & Register's short form subcontract. This subcontractor is used for contracts that are less than $20,000.00.
Purchase Order Agreement
This is a copy of Gibbs & Register's purchase order. This purchase order is used for material purchases.
W-9 Taxpayer Identification Number
This form will need to be provided for Gibbs & Register to enter your company into our Accounting system.
Partial Lien Waiver
This Partial Lien Waiver is required monthly from all vendors that filed a Notice to Owner with G&R. This is form is very similar to the Florida Statute.
Final Lien Waiver
This Final Lien Waiver is required at the completion of the project for all vendors that filed a Notice to Owner with G&R. This is form is very similar to the Florida Statute.
Subcontractor Pay Application Form (Excel File)
All Major subcontractors are required to use the attached spreadsheet when submitting their monthly pay application.
